o Reviewing past incidents where Situational Awareness is involved targeting train and engine employees
o Request input from Train & Engine Employees for ideas on improving Situational Awareness
• Perform Audits of shove movements using flat switching guidelines
• Conduct yard track clearance audits to ensure that employees are following proper procedure
• Conduct safety meetings focusing on crew to crew communication at safety blitzes
• Communicate to employees CRVs that are occurring on CPR based on quarterly evaluations
• Identify high risk incident areas through trend analysis and identify target areas to be communicated to train and engine employees via safety meetings and 1 to 1’s
• Promote Flat Switching guideline strategies through safety blitz and new posters
On June 8 and 09, 2010 the St. Paul Health and Safety committee meeting was held.

INJURIES: To date there have been 3 FRA reportable injuries for the year 2010 in the Twin Cities Terminal.
SAFE EMPLOYEE OF THE MONTH / EMPLOYEE RECOGNITION:
REVIEW OF SOFA AUDITS
MINUTES
Gordon read the rules of the week and the daily job briefing. We went thru old and new business, the Hazardous Condition reports and having an evacuation drill in July. Kevin went thru the Battle Creek plan and how it has been implemented so far. Anneke talked about a couple new LED lanterns and Rob will test them out. Also, we discussed the cleanup days from last month and planned our industry audits for this month.
NEW BUSINESS
Rick brought up the carpeting at the hump is ripped in the Yardmasters tower. The committee is checking into going with a concrete floor to alleviate the problem.
Bob brought up the drinking fountain is clogged at the Island.
Jack would like the committee to look into different styles of clearance markers for the terminal.
Rob would like to test the Breaksticks and the committee decided on the Hump to start and will get feedback.
Rob also brought up the FRA issued a safety advisory on certain FTTX cars, John will check into this how it affects CP Railroad and why weren’t notified.
OLD BUSINESS / ONGOING BUSINESS
Ford Trestle

Jack brought up is the Ford trestle safe? How often is it inspected? When was the last time the running gear-wheels-undercarriage inspected on the engine that derailed? Are there any records? The committee decided to ask Chuck Phillips for an update on the derailment and find out the cause and ask advice from Mike Olsen on 6 wheel trucks going over the trestle. The committee is looking into the cause of the derailment and if any restrictions on the trestle are in effect that we do not know about.

High Visibility Cover on Pole

The pole by the shove lights (1.2.3.4.) still doesn’t have a Hi Vis safety cover on its guide tightening wire. The committee is looking into if there was one put on and if not getting one installed.

Reporting High Wide Cars in St. Paul Yard

Jack brought up he would like high wide loads and high wide empties reported with the TIH-PIH cars in the yard with updates. Adrian is going to add this on the daily job brief for yard crews.

Shane Fielen brought up the pot signal at Lyndale Ave is hard to see till u get a ½ a car away and the pot signal at BNSF University from the West Runner or Loop track is hard to see. The committee was notified both situations are resolved.

Tim brought up there are some pot holes on the road from the cleanout track to the old yard office that need to get filled, also, the back road to the hump is getting rough. The pot holes are filled.

The Hump radio and walkway

Rick brought up the hump radio is still hard to hear for the switchman and the hump walkway slants away from the track at an angle which needs to be leveled out. The walkway is leveled and the committee will continue to monitor the radio.

Wheel Chocks at Ford Yard

It was also brought to the committee’s attention at Ford Yard on the ramps there are wheel chocks from the auto racks laying on walkways and by the tracks. Jay said he will call to get picked up. With all the snow it is not done the committee will continue to monitor. A cleanup day will be set for the spring.
A cleanup day was done in may and the wheel chocks were picked up.

ACCOMPLISHMENTS
1. New paper towel dispenser at the hump.
2. Light above the snubber is fixed.
3. House 2 walkway at Ford Yard is fixed.
4. The ballast was dressed at Humboldt on the west end from the class, receiver switch east between the leads.
5. New Hump lunchroom clock.
6. Cleanup day at Ford, Humbolt, and St. Paul
F.Y.I.
1. Maps of St. Paul Yard and other terminals, including the BNSF and Union Pacific, are now available from any manager on duty. Managers can retrieve and print maps from their computers. Detailed Track Profile Maps from the BNSF are currently available including both BNSF St. Paul, Midway, and Hinckley Subdivisions and have also been posted at the St. Paul Island. Job Aids are also available for Dresser, Roseport, and Peavey. Contact Norb Denzer or any Yard Manager for all your map-related needs.
2. Crews are asked to monitor taxicab performances regarding safety issues. There is a form available at the New Yard Office which should be filled out and turned in to a manager if there are any problems during a taxi trip. Problems with taxi cabs cannot be resolved if these forms aren’t filled out!
3. CPR police should be called when something or someone looks suspicious. Call the City Police for people outside the barriers of the belt crossing. We remind all employees to be on the lookout for suspicious characters or objects, and if suspicious person(s) are seen on or about the property please contact a supervisor, who will contact proper agencies. If you feel safe ask for an I.D.
4. The CPR Wellness Subsidy will cover more than just the annual membership to health clubs. The subsidy (employee only) will also cover smoking cessation, weight loss programs, nutrition counseling, etc… The company will pay half (up to $300 total) of programs like these once per year.
5. Crews are reminded that under Union Pacific rules; we must carry the Timetable and System Special Instructions, and General Orders that apply to the areas we are working. Roseport, 404 Deliveries, Shakopee, etc... Are all affected by this rule.
6. Ask yardmaster about TIH if not informed at the beginning of your shift
7. Yard crews should report grain spills and other debris in the yard to the proper authority so that it can be picked up.
8. There have been a lot of discrepancies with the pay honor systems please read proper S.A.M.
Notice #69 so everyone puts their pay in correct.
9. Everyone should use caution when throwing the Old West and East switch at the west end due
too close clearance with the BN main line.
10. Sometime in the future we will have an emergency evacuation drill.
11. Crews should be reminded there is no smoking in engines.
12. Make sure you isolate your engine when shutting them down.
13. In Twin Cities Terminal the responsibility for the bulletin boards are as follows: Old Yard Office
– Yard Manager; Island Yard Office – Paynesville Sub Road Manager; Humboldt/Shoreham/Ford
– Humboldt Yard Manager.
14. Watch your speed in your cars around St. Paul Yard.
15. The Humboldt Yardmasters will be moving from St. Paul back to Humboldt due to the Battle
Creek project.
16. There is a new notice train crews will have to walk from the hump tower to 1 thru 4 tracks if the hump job is humping.
17. Employees are to be on the lookout for heavy equipment around the old yard office due to the Battle Creek project.

John Shingler was nominated for his quick reaction dumping the air when derailing on the Ford Trestle.

There were 4 audits done with 115 observations.

The committee also talked about the injuries that happened in the last month and discussed the different situations that are going to happen at Humbolt yard when the Battle Creek plan starts.
NEW BUSINESS
Bob brought up train personal cannot take yard air off a track without protection for that track. Also, the rock need’s dressing at Humboldt on the west end from the class, receiver switch east between the leads. Jason is concerned about yard air being left on a track without any blue flag protection established.
Tim brought up the fridges at the hump are old and don’t keep the proper temperature and would like some new ones installed.
Gordon would like the lights at Humboldt checked due to the increase in business expected for the Battle Creek project. Gordon would also like everyone to be aware that not all engineers are qualified from Shoreham to Humboldt.
Shane Fielen brought up the pot signal at Lyndale Ave is hard to see till u get a ½ a car away and the pot signal at BNSF University from the West Runner or Loop track is hard to see. Shane also brought up the hump needs a new lantern battery charger and another slim radio battery charger.
Jack Wrich was wondering if the Hump walkway was leveled out yet and Tim was going to take a look at it.
Jack also brought up is the Ford trestle safe? How often is it inspected? When was the last time the running gear-wheels-undercarriage inspected on the engine that derailed? Are there any records? The committee decided to ask Chuck Phillips for an update on the derailment and find out the cause and ask advice from Mike Olsen on 6 wheel trucks going over the trestle.
High wide loads and high wide empties could be reported on the TIH-PIH cars in the yard with updates.
The pole by the shove lights (1.2.3.4.) still doesn’t have a Hi Vis safety cover on its guide tightening wires yet.

1. Maps of St. Paul Yard and other terminals, including the BNSF and Union Pacific, are now available from any manager on duty. Managers can retrieve and print maps from their computers. Detailed Track Profile Maps from the BNSF are currently available including both BNSF St. Paul, Midway, and Hinckley Subdivisions and have also been posted at the St. Paul Island. Job Aids are also available for Dresser, Roseport, and Peavey. Contact Norb Denzer or any Yard Manager for all your map-related needs.
2. Crews are asked to monitor taxicab performances regarding safety issues. There is a form available at the New Yard Office which should be filled out and turned in to a manager if there are any problems during a taxi trip. Problems with taxi cabs cannot be resolved if these forms aren’t filled out!
3. CPR police should be called when something or someone looks suspicious. Call the City Police for people outside the barriers of the belt crossing. We remind all employees to be on the lookout for suspicious characters or objects, and if suspicious person(s) are seen on or about the property please contact a supervisor, who will contact proper agencies. If you feel safe ask for an I.D.
4. The CPR Wellness Subsidy will cover more than just the annual membership to health clubs. The subsidy (employee only) will also cover smoking cessation, weight loss programs, nutrition counseling, etc… The company will pay half (up to $300 total) of programs like these once per year.
5. Crews are reminded that under Union Pacific rules; we must carry the Timetable and System Special Instructions, and General Orders that apply to the areas we are working. Roseport, 404 Deliveries, Shakopee, etc... Are all affected by this rule.
6. Ask yardmaster about TIH if not informed at the beginning of your shift
7. Yard crews should report grain spills and other debris in the yard to the proper authority so that it can be picked up.
8. There have been a lot of discrepancies with the pay honor systems please read proper S.A.M.
Notice #69 so everyone puts their pay in correct.
9. Everyone should use caution when throwing the Old West and East switch at the west end due
too close clearance with the BN main line.
10. Sometime in the future we will have an emergency evacuation drill.
11. Crews should be reminded there is no smoking in engines.
12. Make sure you isolate your engine when shutting them down.
13. In Twin Cities Terminal the responsibility for the bulletin boards are as follows: Old Yard Office
– Yard Manager; Island Yard Office – Paynesville Sub Road Manager; Humboldt/Shoreham/Ford

Jay read the rules of the week and the daily job briefing. We went thru old and new business, the Hazardous Condition reports and having a safety blitz in the future. The committee also talked about the injuries and human error incidents that happened this year in the terminal.

John brought up two new trains; 270/271 that will originate and terminate at St. Paul yard starting March 15.
Rob brought up on the inside road by the west mini rip switch there is a manhole cover loose and needs to get secured.

3. CPR police should be called when something or someone looks suspicious. Call the City Police for people outside the barriers of the belt crossing. We remind all employees to be on the lookout for suspicious characters or objects, and if suspicious person(s) are seen on or about the property please contact a supervisor, who will contact proper agencies. If you feel safe ask for a I.D.
4. The CPR Wellness Subsidy will cover more than just the annual membership to health clubs. The subsidy (employee only) will also cover smoking cessation, weight loss programs, nutrition counseling, etc… The company will pay half (up to $300 total) of programs like these once per year.
5. Crews are reminded that under Union Pacific rules, we must carry the Timetable and System Special Instructions, and General Orders that apply to the areas we are working. Roseport, 404 Delivery, Shakopee, etc.. are all affected by this rule.
6. Ask yardmaster about TIH if not informed at the beginning of your shift
7. Yard crews should report grain spills and other debris in the yard to the proper authority so that it can be picked up.
8. There have been a lot of discrepancies with the pay honor systems please read proper S.A.M.
notice #69 so everyone puts their pay in correct.
9. Everyone should use caution when throwing the Old West and East switch at the west end due
too close clearance with the BN main line.
10. With the new snow cover there are switch brooms lying around the yard so being observant when walking.
11. John said he would remind managers to call an extra job for the Hump when needed.
12. Sometime in the future we will have an emergency evacuation drill.
13. Crews should be reminded there is no smoking in engines.
14. Make sure you isolate your engine when shutting them down.
15. In Twin Cities Terminal the responsibility for the bulletin boards are as follows: Old Yard Office
– Yard Manager; Island Yard Office – Paynesville Sub Road Manager; Humboldt/Shoreham/Ford
– Humboldt Yard Manager.

Minimum of 24 Industry Audits during the year - at least two per month. Meet with crews that work regularly at industries (i.e. Scrap Iron, MN&S, etc...) before auditing industries at specific locations.
· Increase SOFA Activities
SOFA audits will focus on jobs with high rates of incidents. No minimum requirement for SOFA audits; focus on quality over quantity. Review findings monthly.

Emergency evacuation drills, 2 per year.

INJURIES: To date there have been 5 FRA reportable injuries for the year 2009 in the Twin Cities Terminal.

REVIEW OF SOFA AUDITS
There were 6 audits turned in with 216 observations.
MINUTES
Jay read the rules of the week and the daily job briefing. We went thru old and new business, the Hazardous Condition reports and finalized the plan from the 2010 from the safety framework meeting to submit. Greg talked about the new LED light bulbs being tested on engines around St. Paul Yard and the fuel savings by switching to them. The committee discussed the west end of the old and new westbound switch and options for moving the switch handle away from the BN main line.
NEW BUSINESS
Bob suggested a poster for flat switching at the middle switches watching out for bypassed drawbars would be a good idea.
Rick brought up the hump radio is still hard to hear for the switchman, the hump needs a new paper towel dispenser and the hump walkway slants away from the track at an angle which needs to be leveled out. Also, the light above the snubber is going on and off.
Rob brought up the fourth shove light on four track is dirty or needs a new bulb it is hard to see.
It was also brought to the committee’s attention at Ford Yard on the ramps there are wheel chocks from the auto racks laying on walkways and by the tracks. Jay said he will call to get picked up. Also, there are a lot of potholes on the road from the entrance to the old yard office that need to get filled.
The committee would like to be proactive with the Battle Creek plan when it is finalized.

Jack brought up house 2 walkway at Ford needs to be leveled out from a previous derailment which is causing a tripping hazard. Jack and Jay contacted Greg Nasine from Ford. Still are waiting for a response.

6. Camera at west end of Humboldt is fixed.
7. Participated in a joint situation awareness safety blitz with the river health and safety committee.
8. Had spring cleanup day with safety appreciation cook out.

10. Dirty vents and lights got cleaned at old yard office.

F.Y.I.
1. Maps of St. Paul Yard and other terminals, including the BNSF and Union Pacific, are now available from any manager on duty. Managers can retrieve and print maps from their computers. Detailed Track Profile Maps from the BNSF are currently available including both BNSF St. Paul, Midway, and Hinckley Subdivisions and have also been posted at the St. Paul Island. Job Aids are also available for Dresser, Roseport, and Peavey. Contact Norb Denzer or any Yard Manager for all your map-related needs.
2. Crews are asked to monitor taxicab performances regarding safety issues. There is a form available at the New Yard Office which should be filled out and turned in to a manager if there are any problems during a taxi trip. Problems with taxi cabs cannot be resolved if these forms aren’t filled out!

Minimum of 24 Industry Audits during the year - at least two per month. Meet with crews that work regularly at industries (i.e. Scrap Iron, MN&S, etc...) before auditing industries at specific locations.
· Increase SOFA Activities
SOFA audits will focus on jobs with high rates of incidents. No minimum requirement for SOFA audits; focus on quality over quantity. Review findings monthly.
· TIH / Security Audits / Emergency Response Plan
Emergency evacuation drills, 2 per year.
Participation with CP police in security training and drills.
Crew awareness, location of TIH.

INJURIES: To date there have been 5 FRA reportable injuries for the year 2009 in the Twin Cities Terminal.

REVIEW OF SOFA AUDITS
There were 5 audits turned in with 47 observations.
MINUTES
Jay read the rules of the week and the daily job briefing. We went thru old and new business. Also, we went thru the Hazardous Condition reports and talked about the injuries for the year.
NEW BUSINESS
Rick brought up the chairs in Yardmaster and Switchman offices are bad and need to be looked at.
Gordon brought up the light on pole #40 facing the 43 pocket is burned out.
Jason brought up with flu season approaching there has been some concern for hand sanitation.
After some discussion, the committee would like to have some more hand sanitizers installed by computer terminals, at the hump tower break room, RIT room, break rooms at the old and new yard office, the office at Humbolt, and the FIT room at the old yard office.

Chocks for Explosive Track at the Hump

Rick brought up a set of chocks is needed for the explosive track when engines are put there. Also, there are holes east of the hump tower on the switchman walkway side that need to get filled in. Jay is ordering the chocks and we are checking if the holes are filled.

Emergency Application of Retarders

Jack brought up the emergency application of retarders at the hump from the yard master. Jack believes that the retarders are not getting reset properly by the computer, allowing rail cars to roll free down the hill. Signal Maintainer Mike Ely stipulated if a situation like this happens again the Hump will wait for a Hump Technician before resuming humping to handle the situation.

The radio at the hump is broke and needs to be fixed. When raining or lightning an alarm goes off in consul and the intercom with the top end is also not working. The committee was informed a new radio is on the way. We will check with Manager of Signals and Communication Bob Otis and S & C Supervisor Mike Ely for a timeline when the repairs well be made.

ACCOMPLISHMENTS
1. MU cable hangers got hung around St. Paul Yard.
2. Carpet at hump got cleaned.
3. Timer for hump lights is fixed with a photo sensitive switch.
4. Mike Chalich is handling the unsafe driving of four wheelers and air slips.
5. Hump barricade at snubber in proper position.
6. Camera at west end of Humboldt is fixed.
7. Participated in a joint situation awareness safety blitz with the river health and safety committee.
8. Had spring cleanup day with safety appreciation cook out.
9. The committee had a safety blitz April 13th & April 14th
10. Dirty vents and lights got cleaned at old yard office.
11. Stairs at Dunn fixed.
12. Chock at house 2 at Ford.
13. Industry Audits on Island all customers (May).
14. Industry Audits at Humboldt Yard (June).
15. Lights at Humboldt Yard fixed.
16. New chairs were delivered at Island and Old Yard Office
17. Two safety posters are done.
18. Eye wash stations filled.

22. The committee helped with implementation of FRA 217/218 rules and regulations..
F.Y.I.
1. Maps of St. Paul Yard and other terminals, including the BNSF and Union Pacific, are now available from any manager on duty. Managers can retrieve and print maps from their computers. Detailed Track Profile Maps from the BNSF are currently available including both BNSF St. Paul, Midway, and Hinckley Subdivisions and have also been posted at the St. Paul Island. Job Aids are also available for Dresser, Roseport, and Peavey. Contact Norb Denzer or any Yard Manager for all your map-related needs.
2. Crews are asked to monitor taxicab performances regarding safety issues. There is a form available at the New Yard Office which should be filled out and turned in to a manager if there are any problems during a taxi trip. Problems with taxi cabs cannot be resolved if these forms aren’t filled out!
3. CPR police should be called when something or someone looks suspicious. Call the City Police for people outside the barriers of the belt crossing. We remind all employees to be on the lookout for suspicious characters or objects, and if suspicious person(s) are seen on or about the property please contact a supervisor, who will contact proper agencies. If you feel safe ask for a I.D.
4. The CPR Wellness Subsidy will cover more than just the annual membership to health clubs. The subsidy (employee only) will also cover smoking cessation, weight loss programs, nutrition counseling, etc… The company will pay half (up to $300 total) of programs like these once per year.
5. Crews are reminded that under Union Pacific rules, we must carry the Timetable and System Special Instructions, and General Orders that apply to the areas we are working. Roseport, 404 Delivery, Shakopee, etc.. are all affected by this rule.
6. Ask yardmaster about TIH if not informed at the beginning of your shift
7. Yard crews should report grain spills and other debris in the yard to the proper authority so that it can be picked up.
8. There have been a lot of discrepancies with the pay honor systems please read proper S.A.M.
notice #69 so everyone puts their pay in correct.
9. Everyone should use caution when throwing the Old West and East switch at the west end due
too close clearance with the BN main line.
10. After the snow melted there are switch brooms lying around the yard so being observant when
walking.
11. John said he would remind managers to call an extra job for the Hump when needed.
12. Sometime in the future we will have an emergency evacuation drill.
13. Crews should be reminded there is no smoking in engines.
14. Make sure you isolate your engine when shutting them down.

A paper trail leads to safety

We have been receiving numerous reports of members disciplined following an on-the-job injury. Too often, the injury was the result of a carrier’s failure to correct its own safety hazard or safety violation.
But if there is no paper trail of the safety hazard or carrier safety violation prior to the injury, the injured employee can end up becoming a victim for a second time through discipline.
How do you keep from becoming a victim twice? You write a detailed letter to the carrier describing every safety hazard and every violation of FRA safety regulations encountered – but you must also provide a copy of that letter to your local legislative representative and your state legislative director.
By so doing, you have created a paper trail, preventing the carrier from later disavowing knowledge of the safety hazard or safety violation.
Beyond protecting yourself against arbitrary carrier discipline following an injury, here are three other reasons why you should write that detailed letter:
•The Federal Railroad Administration has a limited number of safety inspectors. They cannot, on their own, find every carrier safety violation. When state legislative directors receive copies of your letter to the carrier, they can refer those safety violations to the appropriate FRA official. By collecting those letters, state legislative directors can also identify multiple violations that should be reported to the national legislative office for handling at the highest levels of the FRA.
•State legislative directors work with lawmakers at the state and federal level to craft legislation leading to new and improved safety regulations. In Illinois, for example, State Legislative Director Joe Szabo convinced the legislature to pass a rail-safety bill based on evidence presented by UTU members about safety hazards. The current federal rail safety bill being considered by Congress includes provisions that would not have been embraced by lawmakers were it not for written evidence provided by UTU members.
•When employees are injured, and they seek recovery under the Federal Employers’ Liability Act, it is not uncommon for the carrier to deny knowledge of the safety hazard or its own violation of safety regulations. But when local legislative representatives and state legislative directors can provide UTU Designated Legal Counsel with copies of such letters, your legal case is strengthened.
Simply calling a carrier’s safety hotline is not enough to protect you, your family or your brothers and sisters from carrier safety hazards and safety violations. Those telephone calls go only to the carrier, which may or may not correct the situation.
Only when your local legislative representative and state legislative director receive your letter can they help you take action to correct the problem and make the workplace safer. If you need help writing the letter, ask your local legislative representative or state legislative director for that help.
Don’t become a victim twice. Write that letter and make sure a copy goes to your local legislative representative and your state legislative director.
